All referrals should comply with referral standard and include in particular:
- Detailed sexual, menstrual and obstetric history
- Detailed past history
- Full medical history including medications
- Copy of partner's sperm count
- Prolactinx3 – only do once if normal
- Thyroid function test
- Testosterone/SHBG/DHEA (if hirsute)
- Partners semen analysis
